Can I list multiple dates of service on one HCFA 1500 claim form?

We do not recommend listing multiple dates of service on a single HCFA 1500 form as this can complicate posting and tracking.

ChiroFusion is set up to generate one claim form for each date of service (DOS). The NUCC allows for multiple dates (grouping) when treatment is rendered on consecutive days. However, this would complicate EOB posting and accounting within the billing module.

To help ensure accurate reporting, the billing system is designed to allot one claim form for an individual encounter date.
